美國精神醫學會在 2013 年 5 月公佈最新版，也就是第五版精神疾病診斷與統計手冊，當中與憂鬱情緒相關的精神科診斷包括：重鬱症、持續性憂鬱症（或稱輕鬱情感障礙症）、適應障礙症，以及經前期情緒障礙症。 基本上，精神科醫師就是根據這些準則來判斷個案是否符合精神疾病診斷。 很多個案到精神科門診就醫時，對於精神科醫師下診斷並不是依靠特定的臨床檢查，往往既驚訝又懷疑；這是因為與診斷許多內外科疾病不同，實驗室檢驗或影像學檢查在精神科診斷所扮演的角色，主要是用來排除精神症狀是因為其他身體疾病而引起的，比如甲狀腺功能低下會引發憂鬱症狀。
