救急外来で対応することが多く、鎮痙剤、鎮痛剤の静注、筋注、座剤などによって痛みを抑えます。 尿量を増加させることも発作の軽減に有効なため、輸液を行う場合もあります。 発作を繰り返したり、消化器症状のある場合には入院治療も適応となります。
画像診断で、自然排石が可能と判断される結石については、水分を多量摂取して尿量を増加させたり、体動によって結石の尿管内下降を図ります。 内服薬による尿管の緊張緩和も併用します。 アルコールや刺激物は、発作を励起することもあり、摂取はすすめられません。
画像診断で、自然排石が困難と判断される場合は各種外科的処置の適応となります。 体外衝撃波結石破砕術（装置）（Extracorporeal shock wave lithotripsy: ESWL）、また細径尿管鏡による内視鏡下結石破砕術（TUL）、経皮的破砕術（PNL）などによって治療します。 現在、開放手術による切石術（結石摘除術）が行われることはほぼ皆無となっています。大きな尿管結石に対しては、最近、鏡視下（後腹膜腔鏡下）の切石術（結石摘除術）が適応されることもあります。
